Cleaners from the United States are required to go through a background check process to join Turno's marketplace. In the state of Georgia, this process has an additional step, which consists of a form you must fill out. Read on to learn more ✍️

If you're from Georgia or have lived there, you must file an additional form called the "Georgia Statewide Criminal History Consent" form. This form is required in Georgia to authorize Accurate Background, the company that runs Background Checks for Turno, to continue the process for you.

⚠️ATTENTION⚠️

If you fail to fill out this form, Accurate may cancel your background check request, and you may need to buy a new one to start again.

After purchasing a background check request and providing the necessary information, Accurate will send you an email to let you know that your signature is required on additional documents. This step is necessary to continue with your background screening.

The email from Accurate will contain further details, but here are the fields you need to fill out in the form:

  • Full name

  • Address (street address, city, state, zip code)

  • Sex (mark one)

  • Race (mark one - if Other, specify)

  • Date of birth

  • Full Social Security Number (partial is not accepted)

  • Physical or electronic/digital signature and date:

    • If electronic, must also include a timestamp and IP address.

    • If digital (drawn with a mouse or stylus), a timestamp/IP address is not required.

You can sign the form electronically or print it out and sign it. Then, upload it with your physical signature on the link on the email from Accurate. For reference, you can find a copy of the form here.
